By signing this document, I release and hold harmless Junior Achievement of Northern Alberta & NWT from any liability.
TEACHER RESPONSIBILITIES
  1. Welcome a JA volunteer into my classroom to present a JA program for which I am registered.
  2. Due to liability, I AGREE TO NOT LEAVE A VOLUNTEER UNATTENDED AT ANY TIME DURING THE PROGRAM PRESENTATION.
  3. Assume full responsibility of classroom management.
  4. Send the evaluation form back within 10 business days following completion of the program, in order to be considered for a JA program the following year

COMMITMENT TO PROGRAM DELIVERY

I understand that the JA program I requested is available at NO COST to me, my students or my school. However, I realize it is a substantial cost to JA sponsors, therefore I understand that there will be a $500.00 administration fee in the event I cancel the program.
